You can change any of your personal details via the internet by accessing Members Online, or by completing the relevant forms below.

Member Change of Details Form
To change your personal details, address or investment strategy, please complete a Member Change of Details Form.

Nomination of Beneficiaries Form

To nominate your beneficiaries please complete a Nomination of Beneficiaries Form.

Additional Contributions Form

To make additional contributions or change your existing contribution, please complete an Additional Contributions Form. Please ensure that your payroll department is aware of these changes.

Benefit Consolidation Form
To transfer your benefits from other superannuation/rollover funds, please complete the Benefit Consolidation Form.

Withdrawal Requests
To make a withdrawal request, please contact the Administrator on 1800 700 885 and a form will be sent to you.

Notification of Tax File Number Form
If you have not previously notified The Corporate Superannuation Master Trust of your tax file number, to ensure you receive the appropriate taxation of benefits, please complete the Tax File Number Notification Form.
